Do you double space the heading?

In an MLA formatted essay do I double space my name, the teacher's name, course and date? I've heard yes and I've heard no, both from what I thought were reliable sources.

u double space everything in MLA format. the entire paper is double-spaced. but dont make extra space for the heading. the spacing of all the lines must be the same.

Yes, you double-space everything.  I know this for a fact. Bookmark the following page for a reference.  It tells you all about MLA format (of course, it also includes instructions for a cover page, which most teachers do not require).
