of what?  If you talking about special education needs, assessments generally refers specific instruments administered by professionals, whereas evaluation refers to the entire evaluation process, and is inclusive of all assessment data.

I'm not really clear on your question, but if you're simply looking for an explanation of what assessments and evaluations are, here it is.

Assessment:  the means by which a teacher finds what a student knows.  This can be done with a typical paper and pencil test, observation or production of an acitivity or project.

Evaluation:  when a teacher uses comparisons (standards, expected grade, etc) to decide if a student has met criteria, very similar to assessment.  The evaluation comes from information recieved in assessment.
