Eye sheet?

My optometrist gave me a piece of paper with a little chart on it so when I look on it I could note any distortions. If anyone knows what I'm talking about, do you also know of a place online I can print it out? I lost the original copy.

Amsler grid , just print the one that you had cz there are many types , good luck
