Sometimes.

It all depends on how major the team is (home league, normally they do. Pros? Not so much) and the coach.

Usually. The players can have whatever number they choose as long as no other player on the team already wears it or if it is a retired number by either the team or the league. Drury and Gomez both had the same number playing for their old teams,  but had to flip a puck to see who got to wear it when they both signed with Rangers. Drury won.
