Immunization record?

i lost my daughter immunization card... they say when i first got it (when she born) that i cannot have a duplicate... is this true.. and now what do i do? she is now 6 yrs old and she has all her vaccines. but is gone now

  1. If you gave a copy to the school then call the school nurse explain the situation and she will get you a copy of it.  If the school does not have them all on file then the only other option is to call all the doctors and get a copy from each one of them for the shot they did give.

    I went to wal-mart and bought one of those collapsing folder files and put all the shot records, birth certificates etc in it it is a lot easier to keep up with that way and small enough for easy storage.


  2. You can have a duplicate, they have to release that information to you.  If they won't, ask for copies of all of her medical records and find a different doc.  If you ask for copies, you will get the immunization records with it.

  3. You can get another one. Just tell your daughters doctor that you need one with all her shot records on it. I just recently got another one for my son as I misplaced his original. All of them from the time she was born are kept on file in the computer system so any doctor can look them all up. No need to visit every doctor

Well generally when you switch Dr. you give the name of the old Dr. to the new one and they ask them for records.  They will then start a new sheet for immunizations and put the time and date on the new sheet.  If you had to give a copy to the school then they will make a copy and give it to you if you need it.

We have the 'Australian Childhood Immunisation Register' so all immunisations are registered no matter which doctor you go to.  Don't you have anything like that?  Or ask your current doctors surgery if they can help you.  I keep a plastic sleeved folder with a copy of my daughter's birth certificate, immunisation records, health info., baptism certificate etc. because I loose stuff too.

  6. I just went through this myself with two children. I called the health department who has a section just for this and they helped me. They told me what I needed to bring and they provided me with a copy of thier shot records!

  7. Your doctor keeps a record of all of the immunization records, I think i lost my kids at least 2times.

  8. Call her doctor and ask for it, they should have updated records. Some states, like Wisconsin, also have a immunization registry where they keep track of all residents' immunizations. If your state has this, the health department can print off a copy for you. Call the doctor she went to most recently- they should have her whole health history on file.

  9. Go to the health department for that city, they may have the records.  Also, contact the doctor because some place may have the records.  These records never should disappear.

  10. Her pediatricians office has it all on file.  You can request a print-out or photocopy from their office.  It is just as good and authentic as the original.

  11. Call the doctor office and ask for a new one.  I've received a couple of the them.

  12. Ask the school for it..they would have a complete copy.
