  1. For a full answer, Wikipedia has a complete article on "maternal death".

While it is a risk, going out on to the highway in a car or getting struck by lightening is far more likely to kill you.

  2. There could be lots of reasons why a women could die from giving birth, but I know that a women is not going to die unless her pregnancy was high-risked.  Something like preeclampsia, toxemia, HELLP syndrome.  Just lots of stuff could go wrong.  Not trying to scare though.

  3. The most common causes of maternal death are hemorrhage and infection.

I agree with the first answer though.  You are more likely to die in a car accident on the way to the hosptial than giving birth.  (I gave birth at a hospital the first time and the second time at home)

  4. It is very very uncommon, escpecially in countries such as the USA, but It can happen from blood clots being loosened and causing a blockage in the heart (especially if mom is put on bed rest and precautions are not taken), hemorrhaging (monitored and treated if in a hospital) or I took care of some children whose mother died from an aneurysm in her brain while giving birth.  It is very rare!

  5. anything in these times is a life risk, you can go to the store and a bus hit you, anything can happen, the possibility  is there but do not worry about it
