Is vanilla extract needed for milkshake?

it's not needed (just makes the milkshake taste sweeter by adding a vanilla flavor to it)

if you have chocolate syrup or strawberries (or other fruit)-you can use it or sprinkle in some cinnamon,nutmeg,or some sugar before blending or just make w/what you have-should taste yummy!!

Nope.  You might blend some ice cubes with your ice cream and milk, but that's really all you need... try experimenting with flavors, though (fruit you might have fresh or frozen, flavorings like maple or peppermint that you might have bottles of in the cupboard, etc), you might be pleasantly surprised!
