You don't say why you split but you need to apologise first and foremost if it was your fault.  I would go with major wooing, flowers, a romantic meal, weekend away etc and be on your best behaviour! Good luck :o)

It depends on what you did and whether or not she can forgive it or she may be able to forgive you but cannot reconcile with you.   It is ultimately up to her but to answer your question the only way to get her back would be to see if she is up to talking.   Then you explain to her what you did and how you had no right to do it and how very sorry you are and if she can see it in her heart to forgive you and get back together.   Advise her too that you are willing to make any kind of reparations necessary to earn her love again.  

Then see what she says.
