Looking for student info question sheet?

I am looking for a question sheet that can help you to get to know your students. Besides the standard name/address/phone, I am looking for something that has personalized questions as well.

I once saw a sheet that asked questions such as "What is your favorite movie? Why?"

"If you could go anywhere in the world where would it be and why?"

It also asked things about extracurricular activities and personal responsibilities.

Can anyone help with some ideas to generate my own question sheet, or direct me to a good one?

Also, I am not sure how old your students are, or what grade they are in, but why not asking your students to write a poem about themselves, using these questions?

Try to "spice it up", it would be much more fun for everyone!
