The R4 is a all-in-one storage device for the DS.  It'll allow you to play videos, music, eBooks, IM, emulators (NES, SNES etc) and so much more.  I can list everything.  There's some good videos here to see what the R4 is capable of.

the R4 can be used to play games for DS, movies, and music. there are also japanees games that are not releaced in english that you can get an english patch for so it's like you have an english verson of the game... the most common example of a game like this is jump ultimate stars.

The R4DS is used to play .NDS Roms on your DS or DS Lite. You need a Micro SD card to be able to properly use it right.
