  1. Yes I had insurance on my prior phone and I bought a Samsung Plum on Ebay and it covered my phone. I even called and cancelled it because I have been through 3 phones, (2 lost, 1 stolen) and the stupid Asurion people are such a pain to deal with I just prefer to buy one on my own. All I did was log on to  mytmobile and change the phone type I was using. The rep at Customer Care said I had a T509 covered under my insurance and I told him to cancel it.

  2. some of ebay's sellers give insurance

  3. The insurance offered by T-Mobile is a plan based service.  You should be able to add it to your plan.  I would not mention how you acquired the phone.  You best bet is to go on the my.t-mobile.com site, change your profile to the type of phone you now have and then log out.  Then log back in and go to "my plan" and change your plan....I would add the insurance there.

That way there are no questions asked, and once you are being billed you are covered.
