Visitors Problem! Please help me?

Question

I am having a website providing Web Outsourcing Services .

I am getting 800 Unique visitors every month. But out of those only 5% fill up either contact us or get-a-free-quote form.

I dont know whats going wrong. Can any one suggest me? For visitor's convenient I have added a quick quote form on every page of my website containing only 4 fields to fill up.

I thought that my contact us & get free quote is too long for visitors to fill up hence i have made it short (4 fields only) and added on every page of my website.

What else i can do?

I want maximum users to fill out my contact us or get a free quote form

Look at websites providing similar services, and see if you get any ideas from those.
