Where do you send it?

I bought a Babolat Racquet on E-Bay (new) but after several months without the overgrip (I'm starting to sweat more often now >< ) the racquet flew off my hand and nailed a a fence while I was serving. Turns out there is a small c**p but big enough to make a weird noise whenever I swing.

The Racquet was the AeroPro Cortex and where should I send it to? and How do I get their number and address?
