Renewing a certificate of naturalization

My mom's certificate of naturalization and social security card was stolen a while ago.

I was wondering how we could apply to renew her Certificate, what would be needed, and how long it would take for the new one to arrive (because she needs the new certificate to get her social security card).

How can I find out where the nearest place to apply is? We live about an hour away from Philadelphia, but I don't know how to find out where the agency is.
