As you can see, I aready have an account with your bank but I would like to open another Firstbank Card credit card account with different account number. The name that should appear on the card is also Mark Shigamisu with $10,000 credit limit.

In your last sentence, correct the spelling of ay to any.  Otherwise, everything looks O.K. to me.
