Requesting leave letter for visit the Makkah.?

I am working one of company as a clerk. one employee requesting leave letter for visit the HAJ, So pls tell me how i can make a request letter?

Tell her you're taking personal time off for religous reasons. Provide a proof of a copy of the airline ticket, just in case they ask.
