Required for adoption?

My fiance and I would like to have a destination wedding. However, the country is requiring that my passport and birth certificate match EXACTLY or, that I provide proof of last name change (it was changed to my stepfather's last name when I was very young.)

They suggested I bring adoption papers showing my stepfather adopted me and I took his last name.

So... where do I start? My stepfather says he would be happy to sign whatever adoption forms I need. Which forms do I need and what is the process? Can I just download some forms online somewhere and file them with a courthouse or such? Oh, and also, I'm in my 30's and have gone by my stepfather's name since I was about 3. Also, I'm currently in the state of Colorado. Thanks!

  1. if this was done in a different city than each other you have to go get your original birth record where you were born and then where it was changed at get that copy you may need to go in front of a judge to resolve to get his aggreeance to the last name trust me my x went thru it just the same way but it was resolved with out a judge

  3. If your last name was "changed" to your step fathers, then there must have been a legal change.  That is the paperwork you need.

    If you just "assumed" his last name without any legal change (unlikely) then you will need to backtrack and do some legal work.

    Fist find out what exactly was or was not done on paper, in court.  Your birth certificate, if you have not changed anything legally, will match your legal name, but then nothing else will.

    Do the research.  Call an adoption attorney and ask the paralegal or assistant for advice.
