Requirements on getting married in jamaica?

Me and my partner are getting married in Runaway Bay Jamiaca in April 09. My partner will be 20 when we get married does she need a parent consent form to get married there .Because she is under 21 . Could any one tell us what we need and cost in total to get married there. we have booked with thomas cook fot the wedding and honeymoon. Eg Blood Test , Fees . + Has any one been to this place or stayed on the Royal Decameron Club at runaway bay could tell us waht excursions there is and the price roughly . thank You Mark
