  1. If good service, 20%...always.

  2. Everything that you ate & drank should be on the bill, also tax and depending on the type of restaurant, gratuity. Most places will let you know beforehand that they have gratuity added to large groups. Some places believe the quality of the service will  warrant a tip greater than 18% so they don't add on gratuity for large groups.

Some places (if they do add on grauity) can make the amount anything they want. I've seen everything from 15% to 50%. If you don't like their gratuity policy, then you can dine elsewhere.

  3. On the bill it will just say the price for everything + tax. It will have a line for you to write your tip in. If you get separate checks it will also look like that.

It's good to tip 15-20%. Being a waiter is a crappy job with poor pay, you often get yelled at by customers and people see it as your fault when there is a mistake made with the food.

  4. 15%. some restaurants include gratuity on bill, some dnt, just check.
