  1. BOTH parents must sign for a chld to get a passport. yhen , if the children are travelling out of the country with only one parent, the other parent must provide a letter of permiaaion, signed and notarized.  If your mother has sole legal and physical custody, she can provide a copy of that court order.  If not, the letter will be needed.  Your mother can just type up a simple letter for him to sign and have notarized stating that he gives his permission for the children named in the letter to travel out of the u.S. with their mother.

  2. You just get your dad to write it and sign it. It needs to include information for getting hold of him in case someone wants to verify the letter.

  3. All your dad needs to do is type up a letter saying you have his permission to travel with your mother and both he and mom need to sign it.  Also it is a good thing to get a notary to stamp it.
