ive been in the states for 3 years and have moved back to canada (im a canadian citizen)

do i need to reapply for OHIP and wait the 3 months?

Yes, you do.  Any time you permanently leave the your province (any province btw) and then move back you have a three month wait.  When you move between provinces, the province you are moving from covers that three month wait period.  (called the interportability agreement between provinces)  Coming back from the USA, there isnt that provincial agreement and you will have that wait.  You will also be required to be in Ontario for the first 153 days of 183 days you register for OHIP.  That doesnt have to be consecutive, but basically dont be gone for more than 30 days in the first six months.

    You should get private insurance to cover your for that three month wait too.

    When you go down to the OHIP office, you are going to need to at least 3 documents with you.  One is your proof of Canadian citizenship (eg Canadian birth certificate, Canadian passport etc), one is your proof of Ontario residency (Ontario drivers license, utility bill **these must show a PHYSICAL address, not box office**), and one other document with your name and signature.  (bank card, credit card, older SIN card)  Btw, you may have to reactivate your SIN number at Service Canada to work here again.
