  1. I am a little unsure of in what situation you are talking about. If professionals are wanting information about a child they would usually want some background on the general development of the child ie: what they can do and  areas that they appear to have difficulties. They will want to know about there expressive language and their understanding of language. Their gross and fine motor skills will need to be dicussed and their general play skils and thier concentration levels. How the child behaves socially with adults and their peers is also useful information. I hope this is of some help.

  2. I am assuming that you are asking this from the viewpoint of a services provider who helps special needs kids, so here are a few of the things the parents are often asked:

    1) What kind of symptoms does the child have?

    2) When did you first notice that the child had delays in speech, language, social skillls, etc.?

    3) Do any of your other children have any handicaps or delays?

    4) How was the pregnancy with the child? Any complications during pregnancy or during delivery?

    5) What areas is your child having the most difficulty in?

    6) What kind of services do you think would benefit your child?

    7) Has you child received a diagnosis from a Developmental Specialist/Child Psychiatrist? What was the diagnosis?

    8) What medications/diets is your child on to help him or her deal with the problem?

    9) How is the child's performance in school? Any areas of concern?

    10) Have you taken the child to physicians to rule out physical causes such as poor hearing or eyesight?

    11) What kind of behavioral problems does your child show? Any particular situations where he/she acts frightened/has tantrums?

    12) What age did he/she start walking/talking, etc?

These are some that are asked...there may be others, depending on who wants to know and what kind of evaluation the individual asking is trying to accomplish.
