Shot record for I485 paperwork...?

my fiance had a medical exam and everything already. he is up to date w/ all his vaccinations. but after his exam he wasnt given anything except for his xray. now while filing the I485 we saw that he needs info on all the shots. how can we get this? does he have to go back to a dr here? or does that only pertain to someone who did not have a phsyical?

No, he needs his records and should have gotten them before coming here.  He does not need them for a fiancee visa, but he needs them for the AOS.  He will have to get them from his home country OR go through having the vaccinations again.  It isn't the end of the world, at worst he needs a few shots.
