There are cases of deaths resulting from lethal doses of a combintion of heroin and Morphine.  as well as Percodan and Morphine. Brad Renfro who was featured as the young actor in John Grisham's  The Client died recently from such an overdose.  There was another case of Morphine overdose  but the immediate cause of death was written as kidney failure.  

It's sad but it's true .  Many cancer patients die from Morphine intoxication in hospices under involuntary euthanasia or mercy killing. In this instance, their death certificates won't say that they succumb to  Morphine overdose but some other diseases such as COPD or  kidney failure or resp failure.
