Sound skips in and out at the beginning only

Question

Everytime i play music or anything that requires sound it keeps skipping in and out or the speed plays fast or slow..but then after a few minutes the speed or sound is fine. Started happening just recently and I've defragmented and did disk clean up so many times hoping it would get fixed. Nothing seems to work. any help???
