You might try slowing down when you talk if this doesn't help try a speech therapist, or and ask if you could have your hearing checked.  sometimes your hearing may have something 2 do with your speech.  What you described 2 me doesn't seem like your hearing but i'd have it checked anyway.

I'd ask mom to make an appointment with a speech therapist.  Most insurance plans pay for at least twenty per year.  Being that you sound more mature than most, you should do well, provided that you do your "homework."  

(I was diagnosed with a speech issue at 23, BTW.)

so your 13 right? In any day, try practicing your word problem for example: (try to say each and every day) "The quick brown fox jumps over the lazy dog" or "The theme of Sony TV is shore of abhor  on a night of twinkie twigs" try them and you"ll be better.
