If a project is in a system design phase and no programming has been started, why would incorporating a requested change add more time and money to the project?

At a minimum, additional time would have to be spent to research the impact of the change - not only could it require modifying the design already in place, but it could also expand the scope of the project, either affecting areas not previously being changed (for an enhancement project) or require whole new programs be fit into the overall design to perform new functionality.
