Question:

TEACHERS--need examples of criteria-based student portfolio forms?

I am a middle school math teacher and I am working on developing student-directed math portfolios that have students track their progress towards meeting state standards. I'm thinking that for each unit, students will be given a form to fill out that states the standards and skills/knowledge needed for the unit....they will be responsible for collecting and citing evidence that they have mastered each targeted standard (using a test or quiz from the unit). If they have not met the criteria, I would also like a self-assessment form that they can use to plan an 'intervention' for themselves (ie. what are YOU going to do to master this skill?). I am trying to make my middle school kids more accountable for their learning and progress.
