The cost depends on the reason why you need it - what the violation was, where you're being required to carry it.  The form gets ADDED to your policy, so you also have to pay for the insurance policy to keep the form.

Bring the paper that requires you to carry the SR-22 to your agent, and they'll get the policy with the form attached for you - or at least get you a quote.
