The best way to take notes on assigned reading (for school)?

how do you take notes on assigned reading for school? i mean should i just summarize the chapters after i read them? Or is there a better way? I know you should write down the important things that happen in the chapter, but i mean just writing random statements about various parts of the story seems terribly unorganized to me....ANY help would be great.
