  1. A quilt is usually a blanket type of bedding with decorative images or stitiching. Designed to be used on top of all the other bedding A duvet (doona in other countries) is inserted into a slip cover and provides warmth.

  2. The duvet -is- the comforter.  The cover is called a "duvet cover", but I often see people using the word "duvet" to mean the cover.  The duvet is used with a cover, because it's filled with down or something, and it's big and bulky and hard to wash.  So it's made with a plain white cover to be used inside something more decorative.

A quilt is thinner, and more washable, and decorative in itself.

Usually, a quilt is quilted -- it's sewn together with lines of stitching, that, when cotton batting is used, have to be quite close together to keep the batting in place.  A comforter is tied instead of quilted, or, if it's quilted, the lines are far apart because the inside doesn't need much holding.  The inside might be polyester or even an old blanket.

Really, duvet and comforter mean about the same.
