  1. If you have a flat painted ceiling, sand lightly and repaint. If you have a textured ceiling: beware! It sounds like you are not allowing the moisture to be pulled out by a fan (or leave a window open after showering).  If you have a textured ceiling, it may fall: happened to me when I attempted to roll new paint on it.  It fell in huge clumps and I had to have a professional come in scrape, retexture and paint.  Now I am careful to leave the bathroom fan on for at least a half hour after every shower.

  2. I would first make sure your roof is not leaking, if it is not then you will know it is from the moister of the shower, i would suggest spraying the spots with bleach to kill any mold or mildew, leave for 5 mins or so then sand it off, once done prime it with a good primer and re-paint, to ensure this does not happen again (if it is not a roof leak) make sure you leave on your bathroom fan for 20 mins to ensure all the moisture is out of the bathroom. This will prevent any mold or mildew in the future. Hope this helps!

  3. Sand off any loose "brown stuff".  Prime first with Kilz primer, which should prevent the mold/mildew from showing through.  Then get a paint specifically for bathrooms, made to resist mold and mildew.  Keep bath well-ventilated.

  4. You might want to get your roof checked for a leak or go into the attic above the shower and see if you can see if any water has been dripping. If there is no leak then spray the ceiling with a bleach and water mixture and let dry. They have cans of kilz it that are for ceilings and spray upwards. Use that and depending on your ceiling then you can paint. If it is a popcorn ceiling the kilz it should be fine otherwise try to match your paint color.
