  1. CO 8-10 stitches and knit 5 rows of garter stitch. On the next row, every stitch, insert the needle as if to knit, but wrap the yarn 3 times instead of one. Pull through as normal. On the row after, knit one of the loops for each stitch. This creates the elongated stitches.

  2. It actually looks like a belt.

I can't find the pattern for it but it's easy enough to figure out.

I'd say cast on 8 stitches using size 8 needles.  Garter stitch (knit every row) for 5 rows, then change needles to the biggest you can find, knit 1 row and switch back to the size 8's for 5 rows.  Repeat until you have the length you want.

I just made a swatch and it did come out looking like the picture.  For the big needle I used a size 13 because it's the biggest I have, but I would go larger to get the result you want.

ADD:  I just made another swatch using the big needles for the cast on and it made a nicer starting edge than the small needles.  It allows space to add fringe when you're done.

Another add:  I actually found the pattern for this and to my surprise, it IS a scarf!  lol
