  1. I always thought that happened later in a pregnancy.   It usually happens most often when a woman is past her due date, and I have never heard of it happening during early pregnancy.  I am not saying that it isn't possible, but I have never heard of that.

  2. yeah it is possible.. some woman have what it is called

an  "immature planceta" . sometimes it doesnt completely

developp or the planceta can be hard so the baby is unable

to survive usually happens in the beginnin weeks.

by hard i mean abrupted placenta (one that has separated from

the uterine wall). this usually causes miscarriages , ask if she

    had uterine bleedin...

but yes it could be possible since premature separation of the placenta, blood is forced into the placenta (or into the uterine wall) which makes it hard.

  3. It means she lost the baby.

For some reason the placenta which supports the developing baby failed.   It happens - its sad.

Your friend should count himself lucky - & next time use contracteption.
