The process to changing my Pakistani passport to my husband's name?

I need to urgently change my passport from father;s name to husbands name so can anyone please tell me where to get this information on how long it takes and what it costs etc?

If you use that application form (Form C) it has the option to make any other changes (ie: name).

It doesn't say how much it costs, you should probably contact the consulate/embassy of Pakistan for more details.
