  1. Since this is a geology question, I suppose the answer should be a scientific one, as well. The Philippines lies on the Philippine plate which is completely surrounded by subduction zones. The Philippine Sea Plate is divided into a western “trapped” and inactive half and an eastern part that formed and continues to grow as a result of westward subduction of the Pacific Plates. The western half is doomed to disappear someday because it is being subducted to the west and north under the Eurasian Plate. This eastern half is composed of several N-S ridges, including some underwater mountain ridges, and submarine volcanoes.

The Philippines is located near mid-oceanic ridges. A long time ago, hot magma spewed through these fissures and as these cooled and solidified in many layers, over thousands to millions of years, formed a new landmass.

These explains why much of the Philippines is covered with volcanoes. Mt. Mayon  is a stratovolcano; which was formed due to this reason.

  2. Do you mean how they were formed? - Geography

The islands are volcanic in origin, being part of the Pacific Ring of Fire, and are mostly mountainous.

As for the start of civilization: - History

The history of the Philippines is believed to have begun with the arrival of the first humans via land bridges at least 30,000 years ago. The first recorded visit from the West is the arrival of Ferdinand Magellan on Homonhon Island, southeast of Samar on March 16, 1521. ♥
