Are you talking about Cauliflower ears ? .. it's when the cartilage  has been broken an beat up so much , it forms the huge chunk on your ears.

Probably his mouth piece.  Fighters often affix it behind but atop of their ear since they don't have any pockets and it keeps them from having to hold it while talking to people, shaking hands, etc.
