Thread gets caught in the s***w behind the thread...

I have a Marie Osmond EM1 Emotions Embroidery Machine. Manufactured by Bernina and they say it is similar to the White 4400. Anyway, it is a basic embroidery machine with a computer connect.
My top thread keeps shredding around the take-up lever. I have taken this machine apart several times, taken out the bobbin and rethreaded about 100 times. On my last take, I noticed that there was thread all wrapped up around a s***w that moves in a circular motion along with the thread take-up lever. The thread must be jumping past the tension disks (yes, the thread is in the tension disks and the tension is on 5.) Anyway. What would make the thread slack jump back far enough so that it can get caught on that s***w/part.
