Three y/o unpotty trains his self?

My three y/o nephew has gone #2 in the potty for a LONG time now but lately he has gone #2 in his underwear. It is like the last 5 days! Two of the times it was after church. My sis-in-law just sees him holding his bottom (the back of his pants). And she doesn't know what to do.

Could be as simple as his diet.  Summer is a great time for eating fresh berries and melons but they are loaded with fiber.  That can have a big impact on a little tummy.  

The other thing to consider is that his mom has become a little more relaxed and confident in his ability to always go to the potty.  She may be forgetting to ask him to go before they leave the house or not reminding him to let her know when he has to go so they can make it to the potty on time.
