Tight f******n in a 6yr old?

can anyone help as i'm very confused. I took my 6yr old to the docs because when he takes a pee it just sprays everywhere the doc sent us to the hospital and the doc there told us that he will need to be circumcised as the f******n is very tight and there is a lot of scaring.He has never retracted his p***s and has never had an infection so how can this be ?Is there any other route we can go down to avoid an op?

 Since all of my sons were born I always stretched their foreskins, they are now 14, 12, and 10 years old and I still pull back their foreskins after they shower just to be sure that they are ok.

My son has the same thing i went to three doc two hostpital and they game me antibiotic cream and striod cream it kills him when he wees
