Has there ever been a case of a player being timed out in international or first class cricket?

Yes, but it's very, very rare. One example was in 1919, in England county cricket when Harold Heygate was dismissed timed out (meaning that he did not arrive at the wicket within three minutes of the previous dismissal) in a match between Somerset and Sussex.
