first off, dont try to lose any more weight after your fitting...

Take pictures of the dress before you hand it to them...make sure you write down if it already had a spot, or loose thread...and write down what they quoted you and what they will do to your dress.

references? how much is it? when will it be ready? if they mess up, they alter for free?  do you steam clean it as well?

sometimes they dont put your dress in a nice garment bag...so ask if they have one or you need to provide one.
