To be a chief, sergeant police, is it hired internally only?

A lot of departments hire outside of their department b/c they don't have enough people who could qualify for Chief or no one wants the position.  As far as sergeant....most are promoted internally from the department.  I guess they could still be hired from outside the dept. from another agency.  That would usually happen b/c they don't want or can't promote anyone internally.
