  1. Wouldn't be my first choice, but you've blacklisted Italy, so how about Ireland?  Dublin is pretty amazing.

  2. I've been a lot of places and don't have a "best" - every place has it's own character with interesting things to see and do. There are a lot of excellent choices though. Prague is a great place to visit - lots of castles, architecture, public art, interesting nightlife, good food, the street performers and artisans on the Charles Bridge are good and there are a lot of interesting day trips (highly recommended - Cesky Krumlov and/or Karlstejn Castle.

Iceland is interesting and somewhat different than the places you don't want to visit. I'm assuming you've already done those.

Denmark is good. The culture is interesting, they're the happiest people in the world according to the recent news survey. Copenhagen is fairly expensive but lots of museums, shopping, and attractions. You can easily take the train to places like Helsingor, Malmo (Sweden), Roskilde - a good hands-on Viking Museum and the tombs of the past kings & queens.

Amsterdam is very tourist friendly. There are nearby beaches, outdoor museums, concerts, the Van Gogh museum, the Rijksmuseum, the Rembrandt House, ... there's always something going on. It's also extremely easy to get around and see the rest of the country using the train and bus system. I go several times every year - late April is really good because the tulips are at their peak around then.

Personally, I like to find local festivals and plan travel around that. What are you interested in? When will you be taking your vacation?
