  1. clean under the rim with CLR. you can get it at home depot.also get scale removerin gallon form, empty your tank half way of water, add scale remover, let sit and flush. using a pipe cleaner, clear the holes under the rim than repete the process. if you get better flow, your on the right track.

  2. The valve in the tank is clogged, or set at too low a level. Look inside the tank.

  3. Pull the lid off the back and flush, see if the lift valve is working properly. Make sure everything appears to be in good working condition. That's where your problem will be.

  4. Remove the tank lid and see if the flapper is completely opening.  That's the first test.

If it is an old toilet, you might have calcium built up in the orifices under the rim. That would restrict the flow from the tank.

You might consider replacing the whole unit.  A new toilet costs around $100 and only uses a little more than a gallon per flush.  Also, some cities offer rebates to inspire people to replace their older water-guzzlers.
