"Tvekan är en smärta alltför ensam för att veta att tro är dess tvilling." Now, I've used a neutral term instead of "his" and "brother", but if you wanna use his and brother in sweedish the text should be like this:

"Tvekan är en smärta alltför ensam för att veta att tro är hans tvillingbror." But that just doesn't sound good, so I'd definetily go with the more neutral one.
