  1. You father needs a passport and visa to travel to Italy.

  2. is your father by any chance from Italy? .and a permanent resident in the US? if you father is from Italy , of course he will not need a visa, it goes without say that he needs a passport with 6 months left of validity at his departure.and his passport of Italy ,if he is from Italy.

    so his travel form?

Green Card holders are entitled to depart the United States on vacation or for work purposes. However, there are limitations on how long an immigrant can stay outside the United States without losing his or her permanent resident status (Green Card). A document called a "Reentry Permit" must be obtained from the USCIS in many cases.

is that what he has? a # Reentry Permit # ?

make sure you father is aware that he CANNOT stay gone for longer than 6 months without prior authorization from the USCIS in advance ,he could loose his permanent resident status.
