The number will be different for each leg of the Triple Crown so I doubt very much that any horse will have won the Triple Crown waering a number 5 in every race.

I believe that they change numbers, but I am not certain....but the only Triple Crown winners I found sporting the number 5 blanket was Count Fleet (won in 1943).

Edit: I believe the horse you are looking for is Ruffian. From the Great Match of 1975 against Foolish Pleasure....which was her last race...and her only loss. Both the race and her life.
