  1. A birth certificate is an official document. If you are not married to a man, you cannot put a man's name in the father section unless that man has signed an affidavit that he is the father of the baby.

Donor conceived babies do not have a father, on their BCs or legally.

If your partner wants to do a second parent adoption, that is a completely different document done at a later time.

  2. Hmm.  On my birth certificate that I had to get a few years ago, it was type-printed and the information was not in "boxes", as birth certificates used to be.  It was more in the form of:

    Name:  Sarah ****

    Mother:  June Cleaver (ha ha)

    Father:  Ward Cleaver

So I think you're really asking a question about g*y/L*****n rights, rather than an adoption question.  Perhaps you should check with your state to see if your child's birth certificate can read:

    Name:  *****

    Mother:  ******

    Mother: *****

I don't think you should have to list your partner as the father, as SHE obviously is not.  But either way, your child can take any surname you'd like him/her to.

  3. You might put unknown since that is the truth you don’t know who the father of this baby is and to put your fiancé’s down would just be very weird even more when the child has to explain why a females name is listed as the father.  Maybe there will be a day when a BC will have  Mother / Mother or Father / father in till that day I think it would be best to leave it unknown. You might even see if you can get mother/ mother listed. Then again I think your partner is going to have to adopt the baby if that is even allowed in the state you all are in.

If you want the baby to have the surname of your partner then give the baby that name. Really it be no different then a mother who has a child out of wedlock but choose to give the baby the father’s name.

  4. Leave it blank, then the child will take the name of whoever carried the child.

  5. Does the birth certificate have to have a father's name? There are plenty of single mothers who don't have the father's name on the birth certificate. I have a friend who was artificailly inseminated (single mother by choice), I don't think she has a father on the certifiacte.

Ask if you can have both of your names on it. These days you would think that there would be a solution for these situations. i.e. just list the parents (not necessarily a mother and a father)

I wonder if they would let you put your partner's name as the "father"?

EDIT - sorry, I didn't mean to imply SHE is the father as replied below. I meant that her name could go were the father's name would have gone. It would be nice if they could replace "father" with "mother".

  6. whatever you decide, make sure it is something that your child wont be embarrassed by.

Talk to you doctor and ask him/her was is customary

  7. you can always leave it blank, or put "unknown"
