Trying to identify and Wild Plant?

I live in Gettysburg PA and I found a plant while out in the woods, I can't identify it. It grows in patches and looks like it a spreading plant that sends out runners. It is about a foot tall. Some plants have one stalk that look similar to that of a dandelion (light green/white and shiney). Other plants split into two branches and at the junction there is a five-pedal flower with yellow parts in the center. The is one leaf at at the end of the stems and each leaf has between 4 and five lobes.

This is probably a long shot by maybe someone can help. Thanks.
